               Section 3: Materials Receiving and Handling



            1.  Do you have a method for quantifying your incoming raw materials?    Yes      No

            2.  How is incoming material packaged?
            3.  Please specify any materials you receive which have a limited shelf life:



            4.  How is scrap or offal handled? Is it disposed or recycled?

               Are there separate receptacles for scrap or offal: What is the size of the receptacles and how often is it
               pulled or disposed?
                 If material is recycled, who is the recycler and what revenue per pound does your company receive for
                 the material? (Provide receipts from the recycler equivalent to one year)

                 What are the specifications of the material? (type or grade, size of scrap pieces, etc.)



            5.  Does any of your received material arrive on pallets?                Yes      No
               If yes, what type and size of pallets?
               What is the final use or destination of received pallets?

               Where (company name and address) are the pallets disposed?

               Does your company pay for the disposal of the pallets and do you receive a revenue for the pallets?


               Does the hauler provide the transportation and in what condition are the pallets?

               How many pallets are disposed per month? (List the quantity of each size disposed.)

               Is there a special receptacle for pallets only? (Please describe its dimensions.)

            6.  How are your supplies received, stored, and distributed throughout the facility?

            7.  How many personnel are utilized in the shipping, receiving, and distribution of supplies?

            8. Please list any materials which are not handled by the conventional material handling system:



               How are these unusual materials handled?

            9.  Describe the extent to which returnable containers, cardboard recycling, and other waste reduction
               measures are used during material handling:

           10.  Is plastic returnable dunnage ever disposed of by your company?
               Where is it disposed?
               Who is the manufacturer (name and address) of the plastic dunnage?
               How much is disposed of per month, and please list the specification of the dunnage? (size, plastic type,
               etc.)
