In full and final consideration (the sufficiency of which I hereby accept and acknowledge)
of the Producer granting me a credit in the Programme (the manner and size of which the
Producer shall determine) (if my Contribution or any element thereof is actually incorporated
in the final edited version of the Programme), I hereby consent to the filming and recording
of my Contribution to the Programme, subject to the following terms and conditions:

1. I acknowledge that the Producer has fully explained the nature and content of the
Programme.

2. I acknowledge that the copyright (if any) and all other rights in my Contribution vest in
and are hereby assigned to the Producer (to the extent necessary by present assignment
of future copyright) and that the Producer shall have the unfettered right to deal with my
Contribution or any part of it as the Producer sees fit. Accordingly my Contribution or
any part of it may be exhibited or otherwise exploited in all media and means whether
now known or yet to be invented (the ‘Media’) throughout the world and universe as
known and yet to be discovered (the ‘Territory’) for the full period of copyright and all
renewals, revivals, reversions and extensions of this period (the ‘Period’). I hereby
waive such rights in my Contribution as a I now have or hereafter may acquire in relation
thereto under Sections 77 and 80 of the Copyright, Designs & Patents Act 1988, and any
so called ‘moral rights’ of authors.
3. I acknowledge that my Contribution may involve my participation in risky or dangerous
activities and confirm that I am aware of the dangers thereof and consent to participate
in such activities. I warrant that I do not suffer from any physical disability or health
condition which may make my participation inadvisable. I agree and accept that I will
participate at my own risk.
4. The Producer will not be liable to me or my legal representatives for any loss or
damange or injury to me or my property caused in connection with the Programme
unless caused by negligence of the Producer.
