5. The Producer may without further consent use my name, likeness, biography,
photographs of me and recordings of interviews with me in publicity for and promotion
of the Programme in the Media throughout the Territory during the Period.

6. I confirm that the subject matter of my Contribution is truthful, that I have not misled
the Producer in any way, that I have disclosed to the Producer all matters which are
relevant to my Contribution, and that my Contribution is not defamatory or otherwise
calculated to bring the Producer into disrepute, and that it does not contain anything
which is an infringement of copyright.

7. I acknowledge that my Contribution or extracts of it may be included by the Producer
in programming not directly associated with the Programme and accordingly consent to
participation in all aspects of programming, that are or could reasonably be associated
with or involved in the type or nature of the Producer’s programming.
8. I confirm that I will participate in the filming of the Programme on the date/s agreed and
acknowledge that the Producer has incurred costs in arranging for the filming of my
Contribution.

9. I agree to indemnify the Producer from and against any and all costs, proceedings,
claims, damages and losses including legal fees, howsoever suffered or incurred by the
Producer as a result of any breach by me of any of the terms of this Release form.
10. I acknowledge that I may receive or acquire confidential information about the Producer
as a result of my Contribution, and agree not to divulge any information about the
Producer’s programming or about the Producer which may have the effect of damaging
the business or reputation of the Producer, its programming or its employees including
but not limited to giving information directly or indirectly to newspapers, television or
other media.

11. I hereby confirm that I shall not seek to enforce any rights to equitable remuneration in
respect of any rental and lending rights which may accrue pursuant to legislation save
insofar as the said legislation provides for separate payments relating thereto to be
collected on my behalf by the relevant national collection agencies (in which case any
claim shall be against such collecting agency) or insofar as the said legislation provides
for separate and specific payment relating thereto to be paid to the Producer for and on
my behalf.
12. This Release form is the whole and only agreement relating to my Contribution and shall
be governed by and construed in accordance with the laws of England and Wales the
courts of which shall be the courts of competent jurisdiction.
