Level 3: Documented agreements on visual controls, labeling of items, and
                             required quantities
Level 4: Reliable methods for housekeeping and inspections documented
                             and practiced
Level 5: Reliable methods for housekeeping and inspections shared

Level 1: No visual measurement of 5S
Level 2: Initial 5S level determined and posted
Level 3: 5S routinely monitored by work group
                             Level 4: Causes and frequency of problems routinely documented;
                             corrective action CA implemented
Level 5: Root causes eliminated; preventive action deployed

Scores from this rating can be shown on a radar chart to display a particular
area’s performance in the 5S rating. An example radar chart for 5S drawn using
Excel’s chart wizard is shown in Figure F.13. The center point designates a score
of 0, and a pentagon ring is shown for scores of 1.0, 2.0, and 3.0 (the maximum
score in this example). This particular workstation achieved a rating of sort
                        1.50, straighten 2.00, shine 1.75, standardize 3.00, and sustain 2.00.

                        Interpretation

These scores are useful for tracking improvement over time and identifying
areas of opportunity. Higher scores demonstrate better implementation of the
                        5S principles.
